Bootstrap Modal with Password confirmation

Hello,

I have a form which when I click submit button the information of the transactions will show before you can really submit the form and do database transactions,
What I want to achieve is that, I want the user to type his/her password in the modal box so that I know that he/she really is person who submitted the form. Any Ideas?
this could be future reference also for all bootstrap users.
